What to Bring to the Exam
Bring any immunization records for each family member seeking entry. This is especially important for pre-school and school-age children. We understand that you may not have these records for each family member. A more complete vaccination record means fewer or no vaccinations during the medical exam.
Bring the following to your medical exam:
During the Exam
Dr. Raffee will assess your overall health. We will test for communicable diseases such as tuberculosis and syphilis. The syphilis test is a blood test that we will take during the exam. The tuberculosis is a skin test done in two parts. You will need to return to our office within two to three days to have the results read.
A Chest X-Ray may be needed depending of results of tuberculosis skin test.
After the Exam
When Dr. Rafffee is satisfied that you meet USCIS criteria, he will complete Form I-693 and seal the form in an envelope for you to submit to USCIS. We will provide you a copy of the completed Form I-693 for your personal records before the doctor seals the envelope. Never open the sealed envelope before you send it to USCIS.
USCIS will not accept the form if the envelope has been opened or altered.
